Service Policy

We are a Group Health provider.  Payment or insurance co-payment is required in full at the appointment time.  For your convenience, we accept cash, checks and all major credit cards.  Please call to discuss our fees or ask any billing questions.

Confidentiality Policy

Patient information is kept private and confidential.  When information is requested for the purposes of patient insurance or medical treatment, it may be released with explicit written permission by the patient.
